*By registering above, I agree to the following statements:

1.  Every time prior to coming to the Indiana Storm Chaser's Convention, I will check to ensure that I do not have any of the following symptoms and have not had them in the last 14 days: chills or a fever of 100.4 or higher, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea.

2.  I will not attend the Indiana Storm Chaser's Convention if I have been diagnosed with COVID-19 or have been suspected of having COVID-19 in the 14 days prior to the convention, or if any member of my immediate household, partner, or any person whom I provide care been suspected of having COVID-19 within the 14 days prior to the convention.

3.  I will ensure that I have not had exposure (more than 15 minutes) within close contact (within 6 feet) with anyone diagnosed with or suspected to have COVID-19 in the 14 days prior to the convention.

4.  I will also ensure that I have not had direct contact with the bodily secretions of anyone diagnosed with or suspected to have COVID-19 (i.e. being coughed on, touching a used tissue, etc.) within the 14 days prior to the convention.

I will confirm that all the above statements are true for me every day I attend the convention.  I agree to perform these checks every day before I enter the convention building. I further confirm that I will not report to the convention on any day when any of the above statements are not true, and will contact Indiana Storm Chaser's Convention Staff to follow the Exposure Plan and ensure the safety of all attendees.
